As a member of the Front Door Team in Staten Island it is my role as the School Transition Coordinator to provide support to families and students who are transitioning from DOE services into adult day services. I work collaboratively with the schools, care coordination organizations and directly with families to ensure that the transition from school to adult life is as seamless as possible. I assist families who are not already receiving OPWDD services with the Front Door process and obtaining eligibility. OPWDD’s mission and vision is to ensure that individuals with disabilities can live rich and meaningful lives in the most integrated environment as possible.
